The Flinders Human Behaviour & Health Research Unit (FHBHRU) at Flinders University has developed a generic set of tools and processes known as The Flinders Program™.
The Flinders Program™ provides a framework for chronic condition management which includes:
- Disease management
- Coordination and
The Flinders Program™enables clinicians and clients to use a structured process for assessment of self-management behaviour, collaborative identification of problems and goal-setting and the development of individualised self-management Care Plans. The tools informing the Care Plan include the Partners in Health Scale, Cue and Response Interview and Problem and Goals Assessment.
The Flinders Program has demonstrated effectiveness in two large Australian Government Funded programs/projects being Flinders Closing the Gap, which provided coordinated and integrated care planning in Indigenous health, and in the Coordinated Veterans Care (CVC) Program, providing coordinated and integrated care planning for veterans.For more information please visit http://www.flinders.edu.au/medicine/sites/fhbhru/self-management.cfm
